These tragedies are not fodder for negligence lawsuits but rather the outcomes of “little accidents” and patchy debriefings. Why does the US health care system – with the world’s best doctors, technology and medicine – lose so many lives? Only 5% of hospital administrators are doctors. The rest are “number crunchers” who run hospitals as businesses. Thus, support for care providers is absent, and camaraderie among hospital staff is lacking.
